Finasteride (Proscar/Propecia)

Finasteride is a medication that is primarily used to treat two different medical conditions: benign prostatic hyperplasia (BPH) and androgenetic alopecia (male pattern baldness). It's sold under the brand names Proscar (for BPH) and Propecia (for hair loss). Both of these conditions involve the effects of dihydrotestosterone (DHT), a potent form of testosterone that is involved in the growth of prostate tissue and hair loss on the scalp.

1. Mechanism of Action

Finasteride works by inhibiting 5-alpha reductase, the enzyme that converts testosterone into DHT. By reducing DHT levels in the body, finasteride helps to: - Shrink an enlarged prostate: In BPH, DHT is thought to contribute to prostate growth. Reducing DHT levels helps decrease prostate size, which can alleviate symptoms like difficulty urinating. - Prevent hair loss: In individuals with male pattern baldness, DHT binds to hair follicles on the scalp, shortening their lifespan and causing them to shrink. By blocking DHT production, finasteride can slow or even reverse hair loss in some men.

2. Uses

  • Proscar (Finasteride 5 mg): Used primarily to treat BPH, Proscar reduces symptoms like frequent urination, weak stream, and incomplete bladder emptying caused by an enlarged prostate.
  • Propecia (Finasteride 1 mg): This lower dose is used for androgenetic alopecia, or male pattern baldness. It helps to slow hair loss, and in some cases, it can promote hair regrowth on the scalp. It's typically effective at treating hair loss on the crown and the middle of the scalp.

3. Efficacy

  • For BPH: Studies show that finasteride can reduce the size of the prostate, leading to improved urinary flow and decreased risk of acute urinary retention. It may also lower the need for surgery.
  • For Male Pattern Baldness: Research has demonstrated that finasteride can reduce the progression of hair loss in 80-90% of men. Around 65% of men see some degree of hair regrowth.

4. Side Effects

While finasteride is generally well-tolerated, there are potential side effects: - Sexual Side Effects: These can include decreased libido, erectile dysfunction, and reduced ejaculate volume. These side effects are usually reversible upon discontinuation of the drug, but in some cases, they can persist even after stopping treatment. - Hormonal Effects: As finasteride alters the hormonal balance, it may lead to gynecomastia (enlarged breast tissue in men), and in rare cases, testicular pain. - Psychological Effects: Some men report mood changes, depression, or anxiety while taking finasteride, though these are less common. - Birth Defects: Finasteride can cause birth defects in male fetuses if taken by women who are pregnant or trying to become pregnant. It's highly teratogenic, which is why women of childbearing age are advised not to handle crushed or broken finasteride tablets.

5. Warnings

  • Pregnancy Category X: Finasteride should not be used by women who are pregnant or may become pregnant because it can harm a developing male fetus. It is also absorbed through the skin, so women should avoid handling the tablets if they are broken or crushed.
  • Effect on PSA levels: Finasteride can lower prostate-specific antigen (PSA) levels, which could mask signs of prostate cancer. Regular screening is still recommended, and doctors may adjust interpretation of PSA tests in patients taking finasteride.

6. Considerations

  • Long-term Use: For both BPH and hair loss, finasteride must be taken continuously for it to maintain its effects. Discontinuation can lead to the return of symptoms (increased prostate size or hair loss) after several months.
  • Not Effective for Everyone: Finasteride is more effective in younger men with less severe hair loss or earlier stages of BPH. It may not work as well in those with advanced disease or severe hair thinning.

7. Alternatives

  • For Hair Loss: Alternatives to finasteride for androgenetic alopecia include minoxidil (a topical solution), low-level laser therapy, and hair transplant surgery.
  • For BPH: Alternatives for BPH include alpha-blockers like tamsulosin and surgical options like transurethral resection of the prostate (TURP).

8. Conclusion

Finasteride is a widely prescribed medication with proven efficacy for treating both BPH and male pattern baldness. While it has its risks and side effects, many users find it a valuable treatment option. It's crucial for patients to discuss the potential benefits and risks with their doctor, especially regarding its long-term use and any personal health concerns.
